Application For Membership

Please download the following forms for your New Recruit Enrollment and return to them completed to our Admin.or Recruit Officer. You must complete this package to participate in any cadet activities.

• The Package Contains :
• Checklist of Documents Required
• CF1158 Recruit Enrollment Form(update fall 2011)
• DND2353 Collection, User and Disclosure of Personal Information
• Cadet Code of Conduct
• DND2571 Basic Health Questionnaire(update fall 2011)- All Recruits must complete this.


• Optional Forms - please see instructions:
  • DND2570 Detailed Medical Questionnaire (if you answered YES to any questions on the DND2571
  • Consent to Disclose Medical Information - Must accompany any completed DND2570
  • Food Services in the Canadian Cadet Organizations - if you answered YES to Question #1 on the DND2571

Please present originals AND submit photocopies of proof of legal Canadian resident status and eligibility for provincial health insurance or equivalent when submitting application.

Acceptable Identification documents include:

• Canadian Passport or
• Canadian Birth Certificate or
• Canadian Landed Immigrant Document or
• Canadian Citizenship Certificate

Plus the following Document

• Ontario Heath Card
